St Wojciech’s Hospital in Poznań opened an innovative drug counselling centre for older patients, who use many medicines to treat their numerous illnesses. This service includes a comprehensive analysis of the patient’s methods of treatment and takes into account all procured medicines, including medicines without a prescription, dietary supplements, herbal medicines. The aim of the analysis is to identify potential interactions:
• medicine-medicine,
• medicine-disease,
• medicine-food.
An experienced clinical pharmacologist, a graduate of the prestigious European Academy for Medicine of Ageing (EAMA), assesses the quality of the pharmacological therapy on the basis of medical documentation supplied by the patient and the list of procured medicines taken by the patient. This assessment will be carried out in cooperation with the patient’s family doctor and its main purpose is to prevent unwanted actions caused by the influence of many medicines on the body. These steps are taken to increase the safety of pharmacological therapy and, in effect, to improve the patient’s quality of life.
